Director, Case Management - CT - Plans, coordinates, and directs comprehensive professional programs which include direct service, consultation, education and advocacy. Addresses health related social/emotional problems of the patients at the Hospital as well as be very visible in the community. Work with in the community targeting larger social issues that may effect health care. Manages the day to day activities of the 19 FTE case management staff.
